We look forward to meeting you at your initial appointment at the Arkansas Sinus Center. Listed below are several things you can do to help us make your appointment as efficient as possible.
At the time of your appointment, a thorough history and ENT exam will be performed. If indicated, a nasal endoscopy may also be performed. This is where some numbing spray is applied to your nasal passages and a small scope is used view inside you nose. If available, we can also review your CT scans with you and explain any findings. Your suspected diagnosis or clinical problem will then be discussed with you, including answering whatever questions you might have. Notes of the office visit will then be sent to your referring doctor (if applicable) and also be available to you online through our Patient Portal.
Our Patient Portal allows you to fill out new patient information and view your records. You can access the portal here. Please call our office for a login and password. We recommend that new patients use the portal prior to your first appointment to provide the required information and save time and paper. We apologize in advance for the number and scope of questions, as they are required by government regulations and insurance companies.
Additional patient forms can be downloaded here for your convenience. To save time, we recommend you print these out, complete them, and bring them to your appointment. You will need a PDF reader, such as Adobe Reader, to open and print the file.
